PACE/PACENET Program Survey Request Letter
Beginning in April 2015, the PACE/PACENET Program will be reaching out to all of its enrollees to update various demographic information such as address, telephone number and emergency contacts. Please complete and return, in the postage paid envelope, the pre-printed update form you will be receiving.

The request for this information has no effect on the enrollee’s participation in PACE/PACENET. It will, however, enable the program administrators to contact enrollees.

The update form also contains a “check box” to learn about other valuable benefits that may be available to enrollees. These benefits include Medicare Extra Help/LIS, Property Tax/Rent Rebate, LIHEAP and SNAP/Food Stamps.
